Please note that you should only fill out the questionnaire after purchasing a design or graphics element from the website. Any questionnaires submitted will be considered a "written contract" with The Shabbee Chick Designs in accordance with our terms of agreement. Please read all terms before submitting my questionnaire. Upon receipt of your purchase and submitted questionnaire, you will receive any email confirmation scheduling your design. If you have not received this confirmation within 24 hours after purchase and submission, please email me or use the contact form on this website.
