Please click on the applicable questionnaire link to fill out your questionnaire to start the design process.  Please note that you should only fill out the questionnaire after purchasing a design or graphics element from the website.  Any questionnaires submitted will be considered a "written contract" with Sprinkles On Top in accordance with our terms of agreement.  Please read all terms before submitting my questionnaire.  Upon receipt of your purchase and submitted questionnaire, you will receive any email confirmation scheduling your design.  If you have not received this confirmation within 24 hours after purchase and submission, please email me or use the contact form on this website.

It is important to note that all questionnaires submitted should be considered "creative inspiration" for the designer to get a "feel" of how you would like your site to look.  Each design is a unique endeavor and the designer reserves the right to exercise her artistic liberties on each design.
